Guidelines for marriage registration application form

It is mandatory to fill the application form in both Hindi and English languages.
To fill in the application form in Hindi, install "Structure Hindi Typing Tool", Google Indic Tool or any other Unicode enabled Hindi typing tool or use the attached Hindi keyboard.
Before filling the application form, make the following preparations-
- Latest photograph in jpg format of size less than 40 KB.
- PDF format of identity certificate, age certificate, and residence certificate in size up to 70 KB.
Please fill in the same address in the residential address whose certificate you are uploading.
Photograph, Identity proof, Age proof, Residence proof are required to be uploaded.
Identity proof, age certificate, residence proof of two witnesses are also required to be uploaded.
Fill the name of your locality/village clearly in the option of Mohalla/Village.
It is also mandatory to upload the affidavit for the bride and groom. The link for the format of the affidavit is given below.
After filling the complete details, please check the filled form completely in the preview, if there is any type of error, correct it by going to the relevant option and once again check the complete details in the preview and secure it completely.
After the complete saving of the application form, the application number and password will be available. Please keep the application number and password safe.
After saving the complete form, select the relevant option for online payment of registration fee.
After payment, take a print out of the “Payment Acknowledgment” and keep it safe.
After filling the application form, along with all your original certificates, you can get the marriage registration done by going to the selected office within 30 days from the date of application on any working day.

Attach (upload) the affidavit only after it is attested by the notary.
Notice: If all the uploaded certificates and affidavits are found to be erroneous, the marriage registration certificate can be cancelled.

विवाह रजिस्ट्रेशन आवेदन पत्र हेतु दिशा-निर्देश

आवेदन पत्र हिन्दी एवं अंग्रेजी दोनों भाषाओं में भरा जाना अनिवार्य है।
हिन्दी में आवेदन पत्र भरने के लिए "संरचना हिन्दी टंकण टूल", गूगल इंडिक टूल अथवा कोई भी अन्य यूनिकोड इनेबल्ड हिंदी टाइपिंग टूल को इनस्टॉल करें अथवा संलग्न हिंदी कीबोर्ड का प्रयोग करें।
आवेदन पत्र भरने से पूर्व निम्न तैयारी अवश्य कर लें-
- नवीन छाया-चित्र 40 KB से कम साइज का जे पी जी फॉरमेट में।
- पहचान प्रमाण पत्र,आयु प्रमाण पत्र, एवं निवास प्रमाण पत्र का पीडीएफ फॉरमेट 70 KB तक की साइज में।
कृपया निवास के पते में वही पता भरें, जिसका प्रमाण पत्र अपलोड कर रहे हैं।
छाया चित्र, पहचान प्रमाण पत्र, आयु प्रमाण पत्र,निवास प्रमाण पत्र को अपलोड किया जाना आवश्यक है।
दो साक्षियों के पहचान प्रमाण पत्र, आयु प्रमाण पत्र,निवास प्रमाण पत्र को भी अपलोड किया जाना आवश्यक है।
मोहल्ला/ग्राम के विकल्प में अपने मोहल्ले/ ग्राम का नाम स्पष्ट रूप से भरें।
वर एवं वधू को शपथ पत्र भी अपलोड करना अनिवार्य है। शपथ पत्र के प्रारूप का लिंक निचे दिया गया है।
पूर्ण विवरण भरने के पश्चात कृपया पूर्वावलोकन में भरे हुए प्रपत्र को पूर्ण रूप से भलीभांति जांच लें, यदि किसी भी प्रकार की त्रुटि हो तो संबंधित विकल्प पर जाकर सही करें एवं एक बार पुनः पूर्वावलोकन में पूर्ण विवरण को जांच कर पूर्ण सुरक्षित करें।
आवेदन प्रपत्र को पूर्ण सुरक्षित करने के उपरांत आवेदन पत्र संख्या एवं पासवर्ड उपलबध होगा। कृपया आवेदन संख्या व् पासवर्ड को सुरक्षित रखें।
प्रपत्र को पूर्ण सुरक्षित करने के पश्चात पंजीकरण शुल्क का ऑनलाइन भुगतान हेतु सम्बंधित विकल्प का चयन करें।
भुगतान के पश्चात “भुगतान पावती” का प्रिंट आउट ले कर सुरक्षित रखें।
आवेदन पत्र भरने के पश्चात अपने सभी मूल प्रमाण पत्रों के साथ किसी भी कार्य दिवस में आवेदन की तिथि से 30 दिन के भीतर चयनित कार्यालय में जाकर विवाह पंजीकरण करा सकते हैं।

शपथ पत्र को नोटरी द्वारा प्रमाणित करने के बाद ही संलग्न(upload) करें।
सूचना: समस्त संलग्न(upload) किये गए प्रमाण पत्र व शपथ पत्र त्रुटिपूर्ण पाए जाने पर विवाह पंजीकरण प्रमाण पत्र निरस्त किया जा सकता है।
